By accepting this Internet Service Agreement:

1. Customer has reviewed and agrees with the terms of this Agreement and the Terms of Service. Customer acknowledges and agrees that this Agreement and Terms of Service may changed in the future but remain binding.

2. Customer represents it owns the Service Location or has permission from the property owner(s) to sign this Agreement, and agrees to gran an easement as provided in section 8.

3. Customer agrees to pay the installation fees, rates, and other service charges identified in the Terms of Service, of which are subject to change.

4. Customer agrees to purchase the Service indicated on the Service Order Summary for at least 12 months.

5. Customer consents to receive autodialed and/or prerecorded calls and text/SMS messages from Elevate, or on behalf of Elevate, including marketing and promotional communications, at the phone number listed on my account. Standard text messaging charges applied by your cell phone carrier will apply to any such text messages. Consent for marketing and promotional communications is not required as a condition to receive services from Elevate, and if you do not wish to consent you can contact Elevate at 877-687-3632 to set up your services.
